ASSUMPTION OF RISK, WAIVER, AND RELEASE OF LIABILITY

I, the undersigned participant, acknowledge and understand that participating in the pickleball tournament involves inherent risks, including but not limited to physical injury, illness, property damage, and other potential hazards. I understand that these risks may arise from my own actions, the actions of other participants, the condition of the playing surface, weather conditions, or other unforeseen factors.

I voluntarily assume all risks associated with participation in this tournament and agree to hold harmless, release, and discharge Sulphur Springs Pickleball Club, Coleman Park, event sponsors, organizers, officials, referees, volunteers, and any other affiliated individuals or entities from any and all liability, claims, demands, actions, or causes of action that may arise due to injury, loss, or damage, including those caused by negligence.

MEDICAL AUTHORIZATION

I authorize the event organizers, medical professionals, or emergency personnel to provide me with medical assistance in the event of an injury, accident, or illness during the tournament. I understand that I am responsible for any medical expenses incurred as a result of my participation.

MEDIA RELEASE

I grant permission to Sulphur Springs Pickleball Club and associated parties to use my name, likeness, photographs, videos, or any other media captured during the tournament for promotional, advertising, or informational purposes without compensation.

CODE OF CONDUCT

I agree to follow all tournament rules, guidelines, and codes of conduct set forth by the event organizers. I understand that unsportsmanlike behavior, harassment, or misconduct may result in my disqualification from the event without refund.
