WAIVER & RELEASE

I, Print Name_________________________________, hereby acknowledge that I have voluntarily chosen to participate in this RGV PICKLEBALL LLC, Pickleball Event/Tournament.

Acknowledgment: I recognize that participating in this event involves risk of injury both minor and severe, due to the physical nature of Pickleball. These risks include, but not limited to, strains, sprains, fractures, heat related illnesses, cardiovascular issues, falls, eye injuries and other possible injuries, (RGV Pickleball LLC promotes the use of protective equipment especially eye protection)!
Assumption of risk: I assume full responsibility for all the risks associated with my participation in this event/tournament, and I hereby release, discharge, and covenant not to sue RGV Pickleball llc, its organizers, sponsors,venue providers and any affiliated individuals from any claims resulting from my participation.
Release: I, for myself and on behalf of my heirs, assigns, and next of kin, release and hold harmless RGV PICKLEBALL LLC, its officers, officials, agents, and or employees, from any and all liabilities, injuries, or damages I may suffer as a direct or indirect result of my participation in the event/Tournament
Medical Treatment: I grant permission to RGV PICKLEBALL LLC, and it’s agents, and representatives to provide or arrange for any necessary first aid or medical treatment in case of injury during the event/Tournament. I agree to assume responsibility for any associated costs.
Media Release: I grant RGV PICKLEBALL LLC the unrestricted right to use and publish photographs and or Video footage of me, or in which I may be included, for promotional and marketing purposes, with out compensation.
Personal Responsibility: I certify that I am physically fit and sufficiently trained to participate in this event/Tournament. I am responsible for my own equipment and enduring it meets safety standards.
Safety/Protective gear and equipment: I understand that I am only allowed to use paddles that are authorized under the USPA guidelines, I will not use UNAUTHORIZED or un approved paddles, I understand Proper shoes for the surface I’m playing on is required and that eye protection is HIGHLY suggested and up to myself to comply with this requirement.
Acknowledgment; I have read this waiver and release form, and I fully understand its contents. I am aware that this is a release of liability and a contract between myself and RGV PICKLEBALL LLC and i sign it of my own free will.
