In consideration of being permitted to participate in this pickleball tournament Eastside YMCA Fundraiser, I, the undersigned, hereby agree to the following waiver and release:

1. Assumption of Risk

I acknowledge that participation in a pickleball tournament involves certain risks, including but not limited to, physical injury, illness, or death resulting from falls, contact with other participants, equipment failure, adverse weather conditions, or other potential hazards. I voluntarily assume all risks associated with participating in the Event, whether known or unknown.

2. Release and Waiver of Liability

I hereby release, waive, and discharge [Event Organizer/Entity], its officers, employees, volunteers, sponsors, and any other affiliated entities (collectively, the “Released Parties”) from any and all claims, demands, damages, or causes of action, which may arise from my participation in the Event. This waiver and release applies to claims of negligence by the Released Parties and includes all claims arising from physical injury, property damage, or other losses.

3. Indemnification

I agree to indemnify and hold harmless the Released Parties from any and all claims, actions, suits, damages, or costs, including attorneys’ fees, arising out of or relating to my participation in the Event.

4. Medical Treatment Consent

I consent to receive medical treatment deemed advisable in the event of injury, accident, or illness during the Event. I understand that I am solely responsible for any medical expenses incurred as a result of my participation in the Event.

5. Fitness to Participate

I certify that I am in good physical health and have no known conditions that may affect my ability to safely participate in the Event. I agree to withdraw from the Event immediately if any health concerns arise during the course of participation.

6. Photography and Media Release

I grant permission to the Event organizers to use any photographs, video, or other media taken of me during the Event for promotional purposes, without compensation.

7. Governing Law

This waiver and release shall be governed by the laws of the State of [State], without regard to its conflict of laws provisions.

I HAVE READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND ITS TERMS. BY SIGNING BELOW, I CONFIRM THAT I AM AT LEAST 18 YEARS OF AGE AND ACCEPT THIS WAIVER FREELY AND VOLUNTARILY.
